look here folks when i saw this trailer,it was a no-brainer i had to check it,out and it certainly delivered,look at the star power he knew it was,going to be good folks already again to,my review of the new series dope sick,[Music],whats going on everybody and welcome,back to the channel today for another,review and today were going to be,reviewing the series dopesick which is,going to be releasing on hulu october,13th now this series has eight episodes,ive only checked out seven uh so i,gotta stay tuned for the finale which i,cannot wait i am seriously whoo i cant,wait to see how this all resolved um,theyre about an hour each and its,youre going to be instantly hooked no,pun intended uh this is a fantastic,series and theres a lot of things i,want to talk about with this so like,bear with me here because i want to um i,want to definitely you know dump out,some information but also give you uh my,appreciation thoughts and reaction to,this series now,when i first was exposed to the opioid,crisis like sure everybody hears about,it we all know that were currently in,it i mean you know what that entitles,you know where you talk about oxycodone,and so on but i wasnt actually exposed,to the like the education the history,the scandal of it until i watched this,series this docu-series that was on hbo,called crime of the century the crime of,the century which is available now and,ive done a review for and that really,opened my eyes about the corruption of,purdue pharma and everything with,oxycontin like it just was a mind,blowing i just couldnt believe the,information that i was you know i was,getting from that so when i saw this,series and i saw that you know uh first,of all the star power and then i was,like okay thats thats a really good,cast and then once i started to see what,it was about i was kind of like oh my,god like now i can go into this,not only with,the knowledge of what this is about but,also like being able to see how theyre,going to bring their own take to it and,the and im guaranteed good performances,that we know that so,it just changed my perception going into,this a lot differently from not knowing,anything to now knowing stuff and seeing,what was it going to deliver and with,that its certainly delivered um so with,that being said which ill go about in,this in a second sure there is a lot of,this being,actual with the characters the events,and and all of that sort of stuff but,there is some fictional stuff to kind of,add some more fluff to it but with,enough case studies out there its even,hard to say that thats even fictional,because they could probably just swapped,a few names and it was just like yeah,that was somebodys story because so,many people were affected by this um,especially in the virginia west virginia,area um to the point that like its easy,to say hey you know we want to interview,you about somebody that was affected by,this we would love to be able to bring,their story to life sure will change,their name and likeliness and whatnot,but theyre this characters truly you,know built around someones experience,and and i i can honestly see that so but,yeah folks this again dope sick it will,be on hulu october 13th now lets get,into it now so,this series,is going to take us from different,uh taking up from different points of,the time now starting back into the mid,80s all the way into basically currently,where were seeing uh different vantage,points of all of the events to the rise,of purdue farmer and how it became such,a like a billion dollar company and the,scandal that was revolt all around it so,were getting,folks both in the dea were getting,folks in purdue were getting folks who,were,addicts were getting uh sales reps,were getting doctors were getting,folks of the town youre getting all,different perspectives of folks who were,basically um affected by the oxycontin,uh,crisis that that its its its,its its by all means exactly what you,think it is it is scary,uh it is sad theres tons of tragedy um,and when you talk about an epidemic like,its really no happy ending to this you,know but its the facts here its the,fact that aussie cotton has ruined a lot,of peoples lives,based on,purdue and their one goal and their goal,was expansion,and money by any means necessary so you,know youre youre going to youre going,to see how america struggled through,this and now robert just focusing on,virginia this affected all of america um,and the opioid addiction and thats what,this is going to uh pretty much expose,you to now,lets go and more in particular into,some of the characters here now michael,keaton he plays this doctor sam finis,who,we see him being a ethical doctor he,wants to just help people hes caring,hes personable um hes trusted but we,see this character go through his own,struggles about buying into it and being,against it and whether it becomes a,personal,journey for him or not you know which i,dont want to tell you we just see this,character really tested about his,beliefs,everything with oxycontin so,michael keaton as you would expect who,also is the executive producer of this,alongside of danny strong,fantastic,i mean i love michael keaton i really,dont think he could do any wrong but,hes fantastic in this because as a,doctor who,um you know,is against it but then maybe starts to,buy into its own we see that emotion the,journey the struggle much like everybody,else who was involved with it who was,suspicious of it but who realized that,oh maybe this could be an answer maybe,this is a miracle drug you know we see,him as a doctor firsthand,battle,this,crisis so very interesting character,there um now,michael stoberg who i mean if you,havent seen your honor youre missing,out on one fantastic series who he,absolutely killed his role in that he,plays richard sackler,he is,the head of purdue pharma,he had one gold as i said and that was,to expand,and that was to make money by any means,and his solution for everything for,anybody who was dealing with any bit of,pain because oxycontin was technically,the successor of ms cotton which with ms,cotton and their patent getting ready to,aspire he had to figure out a way,to take things to the next level to keep,things mark,profitable so he came up with this new,solution and oxycontin and,in order to get people to buy in in,order to make the money you know keep,rolling in and to double and to make,more into a span he had to keep people,addicted he had to get he had to get,people to buy in so his solution at all,times no matter whatever your pain was,was to provide more to them so he made,more he made higher dosage he,distributed more and he got everybody to,buy in everybody he even got the fda,to approve things that they,traditionally wouldnt approve by his,own swindles shall i say and then he got,his sales reps he may i mean he made it,a game like a sport for his sales reps,the more and more you you sell to these,different practices the more money you,get you know and little did he know i,mean i mean he knew but he didnt care,but this was just,the start to the end you know and they,tried to say that this wasnt addictive,that was their whole sales marketing,pitch is that this isnt addictive and,well thats a lot folks so theres that,now caitlyn,dever,is a actor actress who,i have really been fond of ever since,her performance in book smart,i,by all means she is my favorite,character in this and her,performance is top tier,she plays this character betsy who is a,queer young lady who has a love interest,she works in,um the coal mines there in virginia and,her family who just isnt acceptable of,her lifestyle her sexuality you know,that creates tension for her living,so shes making money so she can get the,hell up out of there however she got,hurt,on the job really badly hurt like,no bs she really is hurt,her solution is to go see your doctor,she has to go see,the trusted doctor in town which we know,is,dr,fitness he decides that maybe,this is a means of solution by giving,her oxycontin now

okay so i finally watched dope sick and,as a physician i feel like you know my,reaction to this might be a little bit,different than everybody elses so ill,go ahead and just kind of walk you,through some of the thoughts and,reactions that i had to this uh series,so this series is on hulu um i dont,have any affiliation with them uh or,with any of the people that i mentioned,in terms of financial whatever so this,is just my unaffiliated reaction um i do,think it is worth watching i think that,one of the first questions that popped,up in my mind was wow was this dr fenix,guy was he an actual physician turns out,no uh he is a fictional accumulation of,stories uh the entire hulu series,dopesick is based on a uh investigative,journalist uh,book by a woman named beth macey and,this is dopesick by the same name and,ill include links below to the hulu,series and to the book if you want to go,buy that i do think its an interesting,story that is worth checking out the um,piece by miss macy is actually meant to,be more facts based journalism and the,hulu is a,narrative interpretation of that the,hulu series so lets kind of go through,some of the things that i think are,useful truths that come from this that,arent necessarily um,like a true narrative piece of thing but,just like uh truths about the opioid,concern in general that you might have,heard so first of all uh opioid,addiction is absolutely real now lots of,people have varying degrees of opinions,on how much of a problem it is how,prevalent it is and ill actually,include below some links to different,news articles and stories that think,that maybe dopestick was a little too,heavy-handed and maybe overblowing,things and then some that thought,dopestick really did a good job of,really kind of hammering home some of,the truths that a lot of people are,dealing with with this but,im telling you this as a physician,someone who talks to other physicians,opioid addiction is absolutely real and,it is absolutely a problem whether or,not you want to have arguments about how,prevalent it is all i know is there are,a lot of people who are struggling with,this and for those who are struggling,with this addiction,it is absolutely a real,life-threatening and life-controlling,problem that they have and it is a thing,that,i sure hope they find some freedom from,soon next and i think this is a very,important one because there is a stigma,with substance dependence substance,abuse is i really want to drive home a,very very true fact so,i firmly believe that the majority of,people and i have no actual statistic,for this but this is just my belief from,meeting a bunch of them i firmly believe,that most people who are struggling with,an addiction or a dependence to opioids,did not start that,out of,a desire to have fun or recreation i,believe that most people were started by,some well intending um hopefully,physician or provider or other type who,has the ability to prescribe these,medications and they just became,dependent because it is the nature of,the medicine that it creates dependence,over time in some people so,i believe most of them started with,legitimate pain needs and someone,thought that this might be useful for,them and it turned out to uh take over,their life at some point so i want to be,clear that i do not think that because,someone is dealing with an addiction,that they started with uh some male,intent a lot of these folks are decent,folks who started with someone telling,them this medicine would help them with,the problem they were facing and it,turned out the medicine caused its own,problem now having said that i think,its also important to note that opioid,medications,it is not like they are useless um there,is some indication for use for these,medicines in short-term pain and they,can be very very useful in helping,people get through,immediate issues what we call acute,issues new issues i think what does seem,to be shown in most of the data so far,particularly when you balance it against,the risks,that,there is a,lessened,utility when you start dealing with,chronic pain,because as the longer you take this,medication the,tendency for dependency and addiction,increases as well as the tolerance to,the dose to where it gives you less of,an effect than it used to and that is a,dangerous combination where you start,needing more and more of a medication,that you now require to feel the same,way,and that puts you in,a bad place so that is a thing to keep,in mind with this that over time it,starts to um not do what it used to do,for you the thing that that makes me,think about is theres an episode in the,story and if you are a physician uh,youre a nurse youre somebody who might,who works in the medical universe you,probably have seen this before,but man there was a scene where someone,was describing their current status on,this medication to dr fenix and they,were saying doc its like im sleeping,all the time i cant barely stay awake,and the second i manage to wake up the,only thing i want to do is get another,pill,and that really kind of hit home because,as i watch that scene in hulu and im,sure a lot of the physicians out there,are thinking the same thing man i,pictured some patients that i have come,across and i remembered patient,interactions that ive had and i,remember the the struggle that they were,facing,and um man that that hit real for me it,was one of those instances where,a fictional narrative story can really,drive home some real,real life truths the other real life,truth is and this is uh this is a harder,to deal with because,oftentimes someone ends up with a,dependence developing they end up with,an addiction sometimes their doses get,extremely high to a place that i would,probably call them inappropriate and,then they come up across someone who,says hey i need to reduce that,another,instance that this movie or this uh,series had for me that i pictured,conversations ive literally had with,people was when dr fenix talks about,reducing somebody off a dose saying hey,um you know its been a couple weeks,its time to come off this medicine and,that immediate look of anxiety that,immediate look of,terror in a patients face when you say,hey i think its time to come off this,medication,that is something that if youre a,prescriber you have probably seen before,if youre a prescriber who hasnt seen,that before you probably should start,talking to people about reducing their,opioids but man that is,um its a rough thing because youre,saying to someone i think for your,health we need to take away a thing that,youve started to grow and dependent on,we need to start backing that off and,that can be,really really difficult so when we talk,about these hard situations where,theres a dependence or addiction the,amount of pain thats being relieved is,going down and the idea of withdrawing,is can be terrifying and scary or,physically uncomfortable you kind of,have to start asking yourself and its,always productive right,with the exception of trying to figure,out how to not do this again or how to,not do this with more people but start,asking yourself like well where does the,blame belong and there is certainly,plenty of blame to go around if youre,watching the dopestick movie um ill,include some links below but um quite,frankly obviously the pharma company,bears a lot of the blame for this um,because there was some misleading,um,information being given out and you have,uh physicians who you want to believe,the majority of them were trying to do,good things and prescribing this,medication in appropriate ways to people,in good faith uh because good intentions,dont mean we cant hurt somebody right,um and i think thats one of the,problems with this if you dont have,good information even good intentions,you can hurt people which is one of the,reasons that lying about medication is,such a big deal so i think thats one of,the things you have to look at is pharma,has some issues um clearly and i think,there were some examples of thi

moving on our main story tonight,concerns a public health crisis thats,exposed the failure of american,institutions no not that one not that,one either thats a good point but not,actually this time yep there it is the,opioid epidemic and if youre thinking,hold on didnt you already do a show,about this youre wrong weve actually,done two of them but we thought it would,be worth doing a third installment,tonight anyway for a couple of reasons,first its an epidemic thats still very,much raging exacerbated by both the,pandemic and illicit fentanyl to the,point that last year nearly 70 000,people died from opioid overdoses that,is the highest annual death toll ever,and second you may have seen a bunch of,headlines recently about trials and,settlements taking place around the,country there have been so many of them,it can genuinely be hard to keep track,but we want to focus on just one company,tonight purdue farmer whose rollout of,oxycontin arguably fueled the opioid,crisis we have talked about their,relentless push to sell oxycontin before,but more material has come out in the,last couple of years including this,spectacular glimpse into a 1997,motivational sales meeting for the,companys drug reps,[Music],wow,that was purdues vp of sales performing,an oxy-themed cover of shout and that,has got to be one of the most upsetting,decisions from the late 90s right up,there with give every lonely child an,electronic imaginary friend that will,immediately die and lets set every,sitcom in new york city and make sure,the twin towers are prominently featured,in the transition shots really reminds,people theyre there,purdue pharma is owned by the sackler,family and in recent years purdue and,the sacklers have found themselves being,investigated by the doj and facing,thousands of lawsuits filed by state and,local governments native american tribes,hospitals and individuals and they have,spent years working with an army of,lawyers attempting to negotiate their,way out of all of it which resulted in,two major resolutions including this big,news from last october its being hailed,by the federal government as a major,victory against a company whose drug,oxycontin is part of the public health,crisis of opioid addiction that has led,to the deaths of hundreds of thousands,of americans the maker of oxycontin,admitted to defrauding regulators and,paying illegal kickbacks to doctors its,true purdue has pled guilty to multiple,felonies and it is weird to hear news,that sounds so genuinely good itd be,really nice to get more of that once in,a while like no once in a century,weather events for the next few hours or,billionaire goes to space and dies there,you know something heartwarming,that guilty plea was actually part of a,much larger plan that well get into,later involving the company also filing,for bankruptcy and just this past march,submitting a reorganization plan that,would end sackler ownership and the,thing is this might all sound like a,major victory that the sacklers are,finally experiencing significant,consequences but unfortunately that,could not be farther from the truth so,tonight with the bankruptcies,confirmation hearing scheduled to begin,later this week,lets talk about the sackler family how,hard theyve been fighting to defend,their name the details of the deal that,theyre attempting to strike and why the,whole thing is a bunch of and,lets start with a basic question who,are the sacklers well the sacklers are,the descendants of three brothers arthur,raymond and mortimer who in 1952 bought,the company that would eventually become,purdue farmer and while arthurs branch,of the family sold their stake decades,ago before oxycontin was developed the,heirs of raymond and mortimer both,benefited from and in some cases,actively managed produced boom years of,selling opioids many sacklers either,worked at the company or served on its,board and the massachusetts ag points,out some of them were pretty hands-on,they were in the boardroom they own the,company we have emails and memos that,show the direct control that they,exercised over sales and marketing one,of the sackler family members went so,far as to want to get in the car with,purdue sales reps and drive around to,visit,doctors offices its true and doing a,drug rep ride-along is about as hands-on,as you can get and quick side out there,just spare a thought for the sales rep,in question because sure they too,contributed to the over-prescription of,america and probably led to their own,local mini epidemic but nobody not even,hitler should have to sit in a car with,their boss and talk about what exactly,the weather and then when you run out of,things to say about it do what sit in,silence and then say actually i could go,for a bathroom break right now and have,him say im fine but feel free to stop,and then what you stop and he waits in,the car and then when you get back he,looks things like the fact that he,said i dont know more than a hundred,times now thankfully that deposition has,since been released so you can finally,watch the man in action how much money,has purdue frederick or purdue farmer,made off the sale of oxycontin i i dont,know i dont know i dont know the,answer okay i dont know i i dont know,but,i i just dont know,admit it the guy just oozes charisma,although honestly having actually seen,it i think i prefer the version that we,made,how much money has purdue frederick or,purdue pharma made off the sale of,oxycontin i dont know i dont know sackler,was an officer of the company for years,and even claimed in emails that,developing oxycontin was her idea,although despite that fact when kathy,was called before congress last year and,asked to apologize for her role in the,opioid crisis she didnt exactly do that,i have tried to figure out,was is there anything that i could have,done differently,knowing what i knew then not what i know,now,and and i,i have to say it i cant theres nothing,that i can find that i,would have done differently okay first,that is definitely not an apology kathy,and second come on nearly a year into a,pandemic that is just an unacceptable,at-home setup put your laptop on a,couple of books upgrade your router and,while youre at it one click a ring,light so kathy thats books router ring,lights its the little zoom tricks that,weve all learned thatll help you look,your best as you absolve yourself of,responsibility for a massive public,health crisis,but the sacklers havent just been,appearing at get,into the mind of richard zackler hes,not even though,hes empirically a bad guy,in this drama and in real life,he hes three-dimensional how did you,get into his head well it was really,challenging especially because so many,of the people i interviewed that worked,at purdue pharma,they had such disdain for him so even,the people that knew him spoke about him,in such negative terms so much has been,written about him is so negative,so i was really trying to crack this,code of i just want to know what makes,him tick you know what is motivating him,its got to be more than greed because,he was wealthy before they even came up,with oxycontin so i went to this sort of,drastic idea which was that i did a,therapy session in which i role-played,that i was richard sackler in the,therapy session and i used all the,information that i was able to gather,about richard sackler and it was funny,the therapist is a wife of a famous,screenwriter so shes used to doing this,sort of thing with her husband and it,was within about 20 minutes of me,answering her questions i just really,started to at least get a sense of of,the mindset of someone who does so much,work whose family whose cousins share,the money with him but they dont do any,of the work who has this iconic uncle,that perhaps he has a complex about that,he wants to in some ways maybe even more,successful than and yeah the goal,ultimately was that we wanted all of the,characters in the show to in some way,reflect,pain what is pain its not just the show,isnt just about this criminal company,purdue pharma although a lot of it is,about this criminal company purdue,pharma we really wanted to show their,crimes in many ways i wanted the show to,be the trial that purdue pharma has not,gotten right so we we prosecute them,through the course of the series by,showing their,unbelievably egregious crimes but at the,same time i wanted to be able to,humanize uh richard zackler to to show,okay so what ultimately what is going on,with him and michael you are somebody,who thinks a lot about,whats going on in the country,um the cdc reports that drug overdose,led mostly by opioid deaths were higher,than ever last year,even during the pandemic that almost,more than peak car crashes and gun,violence deaths combined according to,the new york times,beyond,the malfeasance and the people who,actually were in physical pain,what do you think is going on the,pandemic accelerated a lot of stuff and,if you really think logically you start,to piece it together and you think about,it you go it only makes sense,that certain,that certain behavior uh behavioral,things are going to come out of people,there were so many times when i id read,something in the script id either go to,barry levinson or danny and id say,you know the scene plays but this is,like you know it doesnt like is this,too you know,you know,evil and good its on the head and,theyd go think so listen to this and,that didnt give me,statistics like that or they actually,said that or this is what purdue,and then you just kind of cant believe,the obviousness of it you know so in a,way thats not at all shocking to me at,this point these the the statistics are,insane theyre shocking and danny under,a bankruptcy settlement just last month,this is what youre alluding to about,them not getting their uh a trial this,after purdue pharma and the sackler,family are going to pay out 4 billion to,opioid victims and addiction programs,prosecutors still could pursue criminal,charges i mean they still could do that,but under the bankruptcy deal the,sackler family gets immunity against,future civil,lawsuits,thats maddening to a lot of people and,another example of just how the wealthy,get to do whatever they want i think,thats being challenged though isnt it,uh,theyre there theres you know uh some,people are trying to appeal it yeah uh,theres a sense that its probably going,to go through,uh and at the end of the day one thing,that ive discovered in studying this,story for as long as ive been working,on it is that the sacklers always win,they always get away with it and someone,asked me how does that happen and its,just its a one-word answer money you,know theyre able to use their money to,to buy influence to pedal influence at,the highest levels of the us government,and in many ways this story,um its the story of the revolving door,as much as as much it is a story of uh,this this dishonest criminal pharma,company its a story of the swamp,and of the flaws in our systems that,allow uh the ultra ultra wealthy to get,away with a crime so egregious,as misbranding,uh uh a deadly uh a dangerous narcotic,is non-addictive its practically,non-addictive so so this is i think one,of the ultimate examples of of a swamp,story yeah and uh michael uh lastly uh,this isnt the first um,project youve done on this theme i i,remember uh clean and sober yeah uh and,uh also dealing with um dependency,issues and you have a foundation in your,nephews name yeah my sister pam uh uh,uh formed the foundation and i want to,make sure i get it right so i read it to,you its called the michael douglas,shikolone,foundation,and,it goes toward,addiction issues and it goes toward this,other wonderful its im from out just,outside of pittsburgh and so i can read,it right if you dont mind me doing
